These are some older pictures, I have a narrow b-fly but it is wider now than in the pictures. But, I have some what of the same problem, and I hate the double break.

I kept sliding the puck into the pad working my way towards the top of the pad. At about three inches from the top of my thigh rise the puck went right under the pad on both legs. I manipulated my straps and adjusted my butterfly from narrow to wide. I had little success getting the pads closer to the ice. I'm pretty sure this is what the original poster was referring too. I wear a 37" RBK 8K and have worn them for five seasons. They are well worn but in good condition. I am unsure of the reason for this problem and havn't noticed it in the past. I am leaning towards the age of the pad and the possibilty that the top of the thigh rise is not as firm/strong as it originally was. Needless to say I am obsessing over my compromised butterfly.
where is your stick during all of this... if its in the right place you can use ur stick over relying on your pads

Agreed. when i save a puck going fin hole i either deflect it into my stomach (can be risky i guess. and i only do it if the shot is right at my mid section if not it goes into the corner), deflect it into the corner or if its a softer shot absorb it with my stick and just cover it. either way shouldn't rely on your pads to make a fin hole save

Most peoples issue with the V'fly is due to strapping, or toe ties. so check them first.... then the next issue has been pants.

It is a true statement that the stick should be covering your 5 hole, how ever it would be nice to have the security of having squared pads for just incase you get stick lazy or tired lol
